2010 BENEFITS AT A GLANCE
REGULAR FULL-TIME/PART-TIME (minimum 20 hours/week) EMPLOYEES
The following is intended as an overview of benefits for all CANADIAN REGULAR FULL-TIME employees of Entertainment Publications of Canada, Ltd.

Eligibility:

Dental Benefits:

Medical, Dental, Vision, EAP, FSA, Life, LTD, STD, and AD&D: First day of month following 30 days of employment.

RRSP: As soon as administratively possible (1 to 2 pay periods).

Paid Holidays & Employee Referral Bonus: From date of hire.

Bereavement: Up to 5 days off with pay due to death of an eligible family member.

Automatic Deposit: All, or a portion of paycheck may be automatically deposited in up to 3 accounts.

Natural Disaster Compensation:  Up to 5 days paid if employee is personally affected by an eligible disaster.

Tuition Reimbursement: Up to 100% reimbursement for tuition/books. $2,000 max per calendar year. Eligibility is 6 months employment.

Medical Plans are offered through Manulife (additional coverage available in some provinces)

100% Reimbursement

  • Semi-private room charges to daily max $200
  • Out of Canada emergency medical services
  • Prescription drugs
  • Private duty nursing, to max of $10,000 annually
  • Ambulance service to nearest hospital
  • Hearing Aids to $400 every 60 months
  • Room & board in convalescent hospital for rehab

90% Reimbursement

  • Brand name drugs when a generic equivalent is available

80% reimbursement

  • Vision care, to max of $200 + $50 toward eye exam every 24 mos. for covered adults and 12 mos. for dependent children.
  • Certain services provided by licensed paramedical practitioners including but not limited to chiropractor, family therapist, clinical psychologist, podiatrist

A Dental Plan is offered though Manulife

  • Reimbursement based on current dental association fee guide of the province in which service is rendered
  • One recall exam every 9 months
  • 100% reimbursement for basic and preventative services
  • 50% reimbursement for major restorative services
  • Annual max of $1500 for each covered person

Orthodontic services to lifetime maximum of $1500 for each covered person

RRSP:

RRSP is administered by Sun Life Financial

Employee Contributions:  The pre-tax contribution maximum varies based on compensation.  Please refer to the most recent “Notice of Assessment” provided by the CCRA to determine your current contribution limits.

Contributions can be made:

  • By payroll deductions or lump sum
  • On behalf of a spouse

     

Short-term Disability (salary continuance plan)

Basic Life Insurance:

Voluntary AD&D:

67% of you pre-disability basic earnings for up to 17 weeks of disability  from the:

  • 1st day of disability in cases of accident and hospitalization, or
  • 8th day of disability – in cases of sickness
  • May be used to top up your basic earnings if you return to work for half of your work schedule
  • Supplemental benefit for pregnancy related disability topping up your employment insurance benefit to maximum of 67% of your pre-disability basic earnings.

Long-term Disability
66.67% of the first $2500 plus 45% of balance of your gross monthly basic earnings to a maximum monthly benefit of $7500 after the 17th week of disability.

Critical illness plan

Once time payment of $2000 per person regardless of number of illnesses after 30 consecutive days from date of diagnosis – as defined by Manulife – of the following critical illnesses: heart attack, stroke, life threatening cancer

2 times you annual basic earnings to a maximum of:

  • $280,000 without proof of good health
  • $500,000 with proof of good health

Coverage reduced to 50% at age 65 and continues to the earlier of age 70 or retirement if actively employed.  Conversion option available.

Optional Life Insurance:
Available in units of $10,000 to a maximum of $250,000 per person.

Proof of good health is required and conversion option is available.

Accidental Death & Dismemberment:
2 times your annual basic earnings to a max of

  • $280,000 without proof of good health
  • $500,000 with proof of good health

Coverage reduced to 50% at age 65 and continues to the earlier of age 70 or retirement if actively employed.

 

Available in units of $10,000 to a combined employee and spousal coverage of $250,000 max.

Paid Vacation: Eligible for prorated portion of 13 days on 1st of month following date of hire. 13-24 days accrued per year thereafter, based on length of service.

Sick/Personal Days: Eligible for prorated portion of 5 days on date of hire. 5 days per year thereafter.

Holidays: varies by province.

Employee Referral bonus: Payable 3 months after the new employee’s hire date for select positions.
