2010 BENEFITS AT A GLANCE REGULAR FULL-TIME EMPLOYEES The following is intended as an overview of benefits for all USA REGULAR FULL-TIME employees of Entertainment Publications.

Medical, Dental, Vision, EAP, FSA, Life, LTD, STD, and AD&D: First day of month following 30 days of employment.
401(k): As soon as administratively possible (1 to 2 pay periods).
Paid Holidays & Employee Referral Bonus: From date of hire.
Bereavement: Up to 5 days off with pay due to death of an eligible family member.
Automatic Deposit: All, or a portion of paycheck may be automatically deposited in up to 3 accounts.
Disability: Short-term: 60% of base pay beginning on the 8th calendar day of eligible disability for up to 26 weeks. Long Term: 60% of monthly pay up to $15000 after 26 weeks of disability. |

- Co-pay $20 office visits w/ PCP & $35 specialist

- $2,500/$5,000 annual out-of pocket maximum (individual/family)
Aetna Choice POS II (PPO): Provides in- & out- of-network flexibility
In-network:
- $500/$1200 annual deductible
- Co-pay $20 office visits w/ PCP & $35 specialist
- 90/10 co-insurance
- $1,500/$3,000 annual out-of-pocket maximum (individual/family)
Out of network:
- $1000/$2000 annual deductible
- 70/30 co-insurance
- $3,000/$6,000 annual out-of-pocket maximum. (individual/family)

Aetna PPO 2 Annual deductible: $40 Services covered: Preventative: 100%, Basic: 80%, Major: 70% Annual max benefit: $1500

Orthodontic all dental plans cover orthodontic at 60% with $1500 lifetime max. |

Provided through Vision Service Plan. In-Network Benefits: Eye Exam – Free Frames -$120 reimbursed annually
Lenses -$25 co-pay annually Contact Lenses/Exam -$105 reimbursed annually
Flexible Spending Accounts Annual Maximums: Health Care: $5,000 Dependent Care: $5,000
Life and Accident Insurance Term Life and AD&D Insurance: 1.5 multiples of base pay. Additional coverage is available through voluntary programs including supplemental coverage for employee, spouse/domestic partner and children. |
All employees will be automatically enrolled into the plan at 3% pre-tax contribution.
Employee pre-tax contributions to a maximum of 50% per pay. Post-tax Roth 401k contributions up to a max of 50% per pay. All elections will begin within 3 pay periods. Combined contributions from both sources can not exceed 50% of annual earnings or the IRS limit ($16,500 for 2010)
Loan Feature: Borrow up 50% vested balance for 5 yrs or 10 yrs if used to purchase primary residence. $50,000 max, Interest rate: prime+1%.
Natural Disaster Compensation: Up to 5 days paid if employee is personally affected by an eligible disaster. |
Paid Vacation: Eligible for prorated portion of 13 days on 1st of month following date of hire. 13-24 days accrued per year thereafter, based on length of service.
Sick/Personal Days: Eligible for prorated portion of 5 days on date of hire. 5 days per year thereafter.
Holidays: 7 per year
Employee Referral bonus: Payable 3 months after the new employee’s hire date for select positions.
Tuition Reimbursement: Up to 100% reimbursement for tuition/books. $2,000 max per calendar year. Eligibility is 6 months employment.
Employee Assistance Program: Up to 4 free counseling visits per life issue. Dependent care/elder care resource and referral service.
