2009 BENEFITS AT A GLANCE
REGULAR FULL-TIME EMPLOYEES
The following is intended as an overview of benefits for all USA REGULAR FULL-TIME employees of Entertainment Publications.

ELIGIBILTY MEDICAL BENEFITS DENTAL BENEFITS

Medical, Dental, Vision, EAP, FSA, Life, LTD, STD, and AD&D: First day of month following 90 days of employment.

401(k): As soon as administratively possible (within 1 to 2 pay periods).

Paid Holidays: From date of hire.

Employee Referral: From date of hire.

Tuition Reimbursement: Courses beginning after 6 months of employment.

Bereavement: Up to 5 days off with pay due to death of an eligible family member.

Automatic Deposit: All, or a portion of paycheck may be automatically deposited in up to 3 different accounts.

Entertainment Books: 1-2 free books per year. Employees can purchase additional books at a discount.

Sally Foster Products: 50% discount each fall.

Natural Disaster Compensation: Up to 5 days paid if employee is personally affected by an eligible natural disaster.

Two medical plans are offered through Aetna.

Aetna Select (EPO):
  • In-network plan with referrals
  • No annual deductible
  • $20 co-pay for office visits w/PCP
  • $35 co-pay for specialist visits
  • 80/20 co-insurance
  • $2,500/$5,000 annual out-ofpocket maximum(individual/family)
Aetna Choice POS II (PPO):
  • Provides in-network and outof-network flexibility

In-network:
  • $300/$900 annual deductible
  • $15 co-pay for office visits
  • $30 co-pay for specialist visits
  • 90/10 co-insurance
  • $1,500/$3,000 annual out-ofpocket maximum(employee/family)
Out of network:
  • $600/$1,800 annual deductible
  • 70/30 co-insurance
  • $3,000/$6,000 annual out-ofpocket maximum (individual/family)
Two dental plans are offered through Aetna.

Aetna PPO:
Annual Deductible:
$50 Individual/$150 Family

Maximums: $2,000 annual per person
$1,500 lifetime orthodontia

Services covered at:
Preventive – 100%
Basic – 80% after deductible.
Major & Orthodontic – 60% after deductible.

Aetna DMO:
Annual Deductible:
$0 Individual/$0 Family

Maximums:
No maximum

VISION PLAN RETIREMENT SAVINGS PLAN 401(k) ADDITIONAL BENEFITS
Provided through Vision Service Plan.
In-Network Benefits:
Eye Exam – Free
Frames - $120 reimbursed annually
Lenses - $25 co-pay annually
Contact Lenses/Exam - $105 reimbursed annually

FLEXIBLE SPENDING ACCOUNTS
Annual Maximums:
Health Care: $5,000
Dependent Care: $5,000

LIFE AND ACCIDENT BENEFITS
Term Life and AD&D Insurance:
1.5 multiples of base pay. Voluntary programs available for additional coverage for employee, spouse/domestic partner and children.

DISABILITY
Short-term: 60% of base pay beginning on the 8th calendar day of eligible disability for up to 26 weeks.

Long-term: 60% of monthly pay up to $15,000 per month after 26 weeks.

Automatic Enrollment:
All employees will be automatically enrolled into the plan for 3% pre-tax contribution. Elections should begin within 3 pay periods.

Employee Contributions:
Pre-Tax Contributions - 1% to a maximum of 50% per pay period.

Roth 401k Contributions (Post-tax) – 1% to a maximum of 50% per pay period.

Combined contributions from both sources can not exceed 50% of annual earnings or the IRS limit ($16,500 for 2009)

Loan Feature to Access Funds:
Up to 5 years on 50% vested balance to $50,000 maximum. 10 years if used to purchase primary residence. Interest rate: prime rate plus 1%.


You can enroll or change contributions and you can allocate fund balances on a daily basis via phone or internet. Enrollment and contribution rate changes take effect the first available payroll.

Paid Holidays:
7 per year.

Paid Vacation:
Eligible for prorated portion of 13 days on first of month following date of hire. 13-24 days accrued per year thereafter, based on length of service.

Sick/Personal Days:
Eligible for prorated portion of 5 days on date of hire. 5 days per year thereafter.

Employee Referral:
Cash bonus: Payable 3 months after the new employee’s hire date for select positions.

Tuition Reimbursement:

  • Up to 100% reimbursement for tuition/books.
  • Maximum $2,000 per calendar year.


Employee Assistance Program
Up to 4 free counseling visits per life issue. Also has a dependent care/elder care resource and referral service.
